Nurse: Prepare yourself, Doctor! The next patients, Mr and Mrs Tucker, haven´t stopped arguing since they arrived in the waiting room.The other patients are starting to look a bit fed up.
Doctor: Don´t worry, Lily, send them in. It´s certainly not the first time I´ve had to deal with a difficult couple.
Wife: Good morning, Doctor.
Doctor: Good morning, Mrs Tucker. Mr Tucker. Please sit down.
Wife: I´ve finally managed to drag him in to see you with the results of the tests. Here they are.
Doctor: Thank you, Mrs Tucker. (looking at the results) Mmm. How old are you, Mr Tucker?
Husband: I´m 56 and there´s nothing wrong with me!
Doctor: Yes, right. And how much do you weigh, please?
Husband: 94 kilos. I like my food.
Doctor: And your height? How tall are you?
Husband: 1.75m
Wife: He´s been very tired lately, Doctor – he gets very out of breath climbing the stairs. He smokes too much of course, that´s why. Two packets a day, or more!
Doctor: Yes, thank you, Mrs Tucker.
So, you are overweight and you also smoke! Looking at your 24-hour Holter monitor results, I see your blood pressure is always high! And your ECG shows evidence of hypertension. Now, let´s have a look at the results of this abdominal scan… mmm… cholestasis and early signs of aneurysm in the subrenal aorta.
Husband: But I feel fine, Doctor!
Wife: Rubbish! You´re ill! Just look at those results!
Doctor: Well, let´s not talk about being ill, Mrs Tucker, otherwise your husband´s blood pressure will go even higher! What we should talk about is the high risk of cardiovascular problems. Do you have a family history of heart disease?
Wife: Yes, both his parents died of heart attacks!
Doctor: I see. The results of the blood tests indicate hyperglycemia, very high levels of cholesterol, triglyceride and urates. Do you drink much alcohol, Mr Tucker?
Husband: Not very much – some wine with my meals and a drop of whisky with my coffee every now and then.
Doctor: How many glasses of wine a day do you have on average?
Wife: Glasses! Bottles more like!
Husband: Oh, do be quiet, Maud!
Doctor: It sounds like far too much anyway. Do you get much exercise?
Husband: Not enough time for that! I have too much work to do.
Doctor: Well, with a lifestyle like yours, Mr Tucker, you realize you are running the risk of a heart attack or a stroke. Your arteriosclerosis is trying to destroy your arteries. Dyslipedaemia, hypertension, smoking, being overweight, impaired glucose tolerance and probably a lot of stress are all high risk factors. You must try to change your lifestyle and adopt a healthier one. I’m going to send you for a few more investigations, Mr Tucker, so that we can see what damage has already been done.
Husband: I won’t have to go to hospital, will I?
Doctor: No, but you will have to have the tests and take the tablets which I’m going to prescribe for you.
Husband: Is that all?
Doctor: No, you’ll also have to go on a low calorie, low fat, low sugar diet. And I’m afraid you’ll have to give up smoking and alcohol and take more exercise. I’ll also give you something for your hypertension.
Husband: I don’t mind dieting, Doctor, but… cutting out drinking and smoking! That’s too much to ask a man.
Doctor: You must, Mr Tucker, if you want to improve your health.
